Abstract
We report a case of calla positive pre B cell acute lymphoblastic leukemia in a 5yr old male child , presented with bilateral proptosis. This is a case report and literature review. A 5 year old male child presented with acute issues of fever with bilateral excruciating knee pain preceding fall, history of bilateral proptosis for 3 months .Initial workup of hemogram, ophthalmology and thyroid evaluation, imaging (MRI &CT orbit ) were normal . Repeat hemogram showed pancytopenia picture with multiple osteolytic lesions in x ray knee, no lymphadenopathy or organomegaly noted. Repeated detailed evaluation done was normal.Following which bone marrow aspiration study was done. Bone marrow studies revealed hypercellular marrow with 40% blasts and flow cytometry results of blasts positive for CD19,CD10 , cCD79a HLADR, confirmed the diagnosis of CALLA POSITIVE PRE B- CELL ACUTE LYMPHOBLASTIC LEUKEMIA (ALL).Proptosis is usually associated with acute myelogenous leukemia , and rare in ALL especially when they precede the systemic disease and should be borne in mind when evaluating for proptosis .
